Recheck for left wing (metacarpus)to recheck radiographs to evaluate healing.

Radiography today shows improved healing of the major metacarpal bone with visible callous bridging today for this fracture.

Palpation of the left metacarpus indicates very stable healing for both major and minor.


Plan:
Move this owl to larger group flight enclosure 31. Plan is to give this bird a couple of weeks to build muscle strength with flight and then hopefully release.

Decided to try in a larger habitat - I worry more about him hitting the end of that wing on the side of the condo, and think he could benefit from more exercise.

Veterinary Exam 3/20/24:

Patient was placed under brief Isoflurane anesthesia for recheck imaging.

Radiograph (VD)view taken of left carpus.
Radiograph findings today are similar to findings from five days prior. There is callous formation and bridging of the minor metacarpal bone. The major metacarpal bone continues to have a 1 mm radiolucent gap between the proximal and distal fracture pieces.

On palpation both metacarpal bones palpate very stable.
PT performed on left wing with good extension of left elbow and left carpus.
The soft tissues overlying the area and dermis looks very good, healthy.

Replaced light padded Sam Splint support with modified vet wrap bandage.

Plan:
Recheck radiograph again in 5 days.

Anesthetized for wound care and PT. The wound on the ventral aspect of the L metacarpus is almost completely healed /filled in.
Radiographs still show a significant gap between the two fracture fragments, although it feels solid.
Carpus a bit stiff but loosened up quickly with PT.
Was debating trying to tape splint to wing but leave it unbandaged, ultimately decided to keep w/ current plan. PT is going well and we need better signs of radiographic healing.
P: Continue PT Q 2-3 days. Recheck radiographs in 5 days - then in 10. Once the fracture line is filled it we can try to leave the bandage off. Dr Gardner

VET NOTES
Anesthetized for PT and radiographs.
On radiographs - there is a gap in the major metacarpal at the fracture site. It feels solid but is concerning. The edges are not extremely sharp, but not as rounded as I would hope for this stage of healing.
The carpus is also rather stiff - could not completely extend. While doing PT, I noted that there was still a fairly significant scab over the initial surgery site. I removed it, and it extended relatively deep (could not confirm it went into bone but that is the suspicion). Cleaned well w/ Betadine and flushed copiously, then applied Silver gel and re-bandaged.
I suspect that this was an infectious plug that could be affecting the healing of the fracture. I am hoping that with it removed that fracture line can finish filling in.
P: Continue PT EOD, wound care as well (sterilly). Add Enrofloxacin and Clindamycin (made them SID). Recheck radiographs in 1 week (unless Tracey decides to do them Wednesday). Dr Gardner

Veterinary Exam, Left Carpus/Metacarpal Fracture with IM pin placement previously, Pin has been removed previously:

Patient placed under brief general Isoflurane anesthesia at 3% via face mask.
The existing bandage was removed. PT performed with left carpus and elbow. Good ROM achieved.
The medial surgical incision is fully healed now and the bruising has resolved that was previously present. All dermal sutures were removed and area gently cleansed and small amount of Silvadene ointment. Telfa pad placed and then layer of cast padding then vet wrap into figure of 8 bandage.

Patient recovery from anesthesia was uneventful.

Under GA took radiographs to evaluate pin and fracture. There appears to be solid callous and fairly good alignment. For metacarpals, the healing is usually prolonged due to poor blood supply - but the pin needs to come out because it is impeding extension of the carpal joint.
Moved up to surgery, cleaned the pin site exit, and pulled pin. It came out easily. The callous felt very solid and there was good extension at the carpus. However, given past bad experiences with carpal fractures made a light splint w/ Sam splint and placed a light figure-8.
Continue PT every 2-3 days. Would leave wrapped at least a week, but we can do radiographs next week to see how the callous looks now that the pin is out. Dr Gardner

Several chew tabs missing but bandage nicely in place.
Removed carefully, pin appears to be stable, did not remove the tape over the pin end because it was well-adhered and I didn't want to disrupt the pin.
Did PT on elbow and carpus. The carpus is a bit stiff but loosens up, to perhaps 3/4 normal ROM.
Bruising and discoloration is improved immensely and it is just a bit yellow. There is a thick callous forming.
For now I agree with tentative plan to remove pin next week but continue to leave bandage and do PT. Will change pending radiographs.
Rewrapped w/ padding, put some triple antibiotic under the tape around pin exit. Dr Gardner

Veterinary Recheck, Left Wing/Metacarpus Fracture Repair:

Patient was examined under brief general anesthesia. Mask induction and maintenance with Isoflurane.

The existing bandage was removed from left wing. Examination of the medial aspect of the left metacarpus shows improvement in terms of tissue health and appearance. Much less bruising and the previous bruising is resolving.
The incision is intact and appears to be healing well.
The pin palpates stable and in place.

PT was performed on left wing and left patagium.
ROM measured at elbow 140 degrees, wrist at 165-170 degrees.

Cleansed the incision site gently with sterile saline, SSD ointment, Telfa pad, cast padding and vet wrap. Duct tape tabs placed to prevent chewing of bandage.

Plan: Continue PT every 3rd day on left wing. Recheck incision in 7 days.

Continue oral medications through next week's recheck.

BARP, very fiesty and bitey

Bird did bite on chew tabs a bit.
Under anesthesia, removed bandage to assess surgery site. Very bruised, Telfa is bloody, but the sutures sites look great underneath and there is no current oozing.
Pin is in place.
Radiographs show great pin placement and alignment of major metacarpal. The pin fills about 3/4 of the bone. There is a larger fragment dorsally, but it is hard to tell where is it in the skyline because it was not totally straight.
Did PT - the carpus was very stiff, and the pin does keep it from being fully extended, which I assume is why Dr Tracey wants to only leave the pin in for 2-3 weeks.
Wrapped well w/ figure-8 and padding.
Added Pentoxyfylline due to poor blood supply in distal limb.
Continue as planned. Dr Gardner

Procedure - Fracture repair - metacarpus
Performed by - Dr. Ritzman
Surgery time - 45 minutes

Surgery Report, Metacarpal Fracture Repair: 

Patient was placed under general anesthesia and intubated. 
The left metacarpal area was gently plucked and cleansed.
The open wound already present on the medial (ventral) aspect
of the carpus was utilized for the incision. A longitudinal incision
was made using the open wound site along the fracture site. The
major and minor metacarpal bone fracture ends were identified.
 
The left wrist was fully flexed and a 0.035 mm sterile IM pin
was driven into the proximal fragment of the major metacarpal
bone and then driven to exit at the wrist joint. The distal end
of the pin was then aligned into the distal fragment of the major
metacarpal bone fragment and aligned. I removed two free butterfly
bone fragments from the fracture site prior to alignment. 
Pin length was measured and palpated for placement. 

The distal end of the pin was cut to leave approx. 1 cm of pin
length for future removal. 

Soft tissue closure made with 4-0 PDS in simple continuous pattern
for SQ tissues and then simple interrupted pattern for dermal
layer. A small amount of tissue adhesive was applied to incision
as well.

Telfa pad and Teguderm and then light figure of 8 bandage placed
over the left carpus. 

Post operative radiographs were not taken today. Plan is to take
those at first PT session. 

Patient recovery was uneventful but patient had some audible
congestion after ET tube was removed. 

Treatment Schedule for PT: Start PT in 48 hours and every 48
hours for next two weeks. Anticipate pin removal in approximately
2-3 weeks.
